Fatality Crash

RED OAK, Iowa — A Red Oak woman was killed and another Red Oak resident was injured in a two-vehicle crash Tuesday evening at Highway 34 and 4th Street in Red Oak.

According to the Iowa State Patrol, the crash happened May 5, 2026, at 5:15 p.m. A 2008 Toyota Scion driven by 67-year-old Helen Louise Slavinski of Red Oak was traveling north on 4th Street. A 2015 Peterbilt semi, driven by 37-year-old Kyle Olive of Corning, was traveling east on Highway 34.

The Iowa State Patrol report says Slavinski failed to yield to traffic at the intersection, entered Highway 34, and was struck by the eastbound semi. Slavinski’s vehicle came to rest in the north ditch.

Slavinski was transported by Red Oak Rescue to Montgomery County Memorial Hospital, where she died from her injuries. A passenger in Slavinski’s vehicle, 28-year-old Kaylin Marie Carlson of Red Oak, was also injured and transported by Red Oak Rescue to Montgomery County Memorial Hospital.

Both Slavinski and Carlson were wearing seatbelts, according to the report. The semi was owned by Peterson Farm Trucking LLC of Corning. No hazardous materials were involved.

The crash was investigated by the Iowa State Patrol. Red Oak Police and Montgomery County agencies assisted at the scene.
