Clarinda Regional Health Center’s Transitional Care Program Helps Patients Recover Closer to Home
When patients finish an acute hospital stay but still need skilled nursing, therapy, or medical support before they can safely return home, Clarinda Regional Health Center offers a local option through its Transitional Care Program.
Ryanne Shatava, BSN, RN, works with the program, which is also known as swing bed. She has been with the hospital for about a decade and has helped grow the service over the past two years. Shatava says she cannot imagine doing any other job. She started as a personal service attendant in assisted living while studying to become a teacher, then switched to nursing because she wanted to help people get better.
Eligibility generally requires a qualifying acute hospital stay. For Medicare patients, that typically means at least three nights of acute care; private insurance rules can differ and often involve prior authorization. People who know they are facing surgery or have questions can contact Shatava or their primary care provider in advance. Case managers and social workers at other hospitals can also refer patients.
Anyone wanting more information about the Transitional Care Program can call Shatava directly at 712-542-8292, call the main hospital number at 712-542-2176 and ask for transitional care, or visit the Transitional Care section on the Clarinda Regional Health Center website.
